Sample Model Release

In exchange for a consideration which has been mutually agreed upon, I [model name], hereby assign all rights to the use of photographs made of me on [date] by David Glenn Rinehart, herein referred to as the photographer.

These assigned rights include copyright, the right to copyright, the right to exclusive use and/or benefit resulting from the use and/or sale of these photographs, the right of reproduction, whether in whole or in part, and the right of unrestricted use of these photographs for any purpose whatsoever, including the rights of artistic, editorial, commercial and/or advertising uses. These assigned rights will and do extend to the photographer, his licensees and/or assignees, successors in interest, legal representatives, heirs, anyone acting in his behalf, or in the behalf of his licensees and/or assignees. Included in these irrevocable rights are the rights of retouching, blurring, alteration, digital alteration or any other technique deemed appropriate for artistic, editorial or reproduction purposes, in any form whatsoever, in any publication whatsoever, whether in print or in electronic/digital form.

I waive the right to inspect or preapprove the photographs or finished version(s) incorporating the photographs and/or any written copy that may be created and/or included and appear in connection therewith, other than at the initial photo proof stage.

I hereby release and hold harmless the photographer and/or his representatives, as set forth above, from any liability by virtue of any blurring, alteration, distortion, optical illusion, or use in composite form that may occur during the taking of or reproduction and/or publication of these photographs, or in any processing, unless malicious intent can be shown. I waive any claim I may have based on usage of the photographs or works derived therefrom, including but not limited to claims of invasion of privacy or libel.

The consideration which has been mutually agreed upon will consist of permission to download and print one image for personal use from the Internet address http://www.stare.com/art/confess/.

I affirm that I have read this release and that I full understand and agree to its terms. I also affirm that I am eighteen years of age or older, and that I am fully able and competent to enter into this agreement and contract.
